Do you need just one, or do you need to be able to get lots of them?
I don't know of any personally, but it's probably best to just buy an old network card with one on. I just had a quick look and you seem to be able to get them from the US for under $10 on eBay.
If you needed more, you could probably get somewhere with a CNC machine to cut them out of sheet metal for you, then you'd just have to bend the top over yourself.
