In your pin connections, you write:
VBAT -- red -- PIN4 GND
GND -- grey -- PIN5 +5V (DC).
Should it not be:
VBAT -- red -- PIN5 +5V (DC)
GND -- grey -- PIN4 GND.
as you has drawn it.
